Varicocele Surgery Recovery: Healing Process & Guidelines

Postoperative Varicocele Period: Misconceptions and Golden Rules

Microsurgical varicocele surgery is an extremely specific, delicate operation that directly involves the testicular vascular network. No matter how perfectly the operation is performed, the rules the patient must follow after varicocele surgery play a major role in the comfort of the recovery process, healthy blood supply to the testis, and the long-term success of male infertility treatment.

Based on the statistics of our clinic, which has performed over 6,000 microsurgical operations in Turkey, and international guidelines, we will cover the frequently asked questions and the golden rules to follow during the recovery period in detail in this guide.

1. The First 24 Hours After Surgery and Hospital Discharge

Varicocele surgery generally takes between 1 and 2 hours. When you are transferred from the recovery room to your private room after the operation, you must not eat or drink anything for the first 2-3 hours. Liquid foods will be provided after the effect of anesthesia completely wears off.

  • Getting Up: You can take your first walk accompanied by a nurse or caregiver approximately 4-5 hours later. This walk should be short and not tiring.
  • Discharge: Patients with no observed risk factors are discharged on the evening of the same day or the next morning.
  • Travel: Patients coming from out of town or abroad can return home by plane or car (preferably not driving yourself, but having a relative drive) after discharge. It is important to take frequent breaks and elevate your feet during long journeys.

2. How Long Does Pain Last After Varicocele Surgery? Is It Normal?

One of the most frequent questions we receive from patients is "Will there be pain after surgery?". The microsurgical incision of approximately 2-3 cm made in the groin is very small.

First few days: A mild tingling and pulling sensation at the incision site is completely normal. Sensitivity in the testicles or groin area, and slight stinging when sitting down, standing up, or coughing may occur. This is controlled in a very short time with standard pain relievers (analgesics) prescribed to you.

Non-chronic Pain: A sharp, stabbing, or unbearable pain after surgery is definitely not expected with the microsurgical method. If there is excessive swelling, intense redness in the surgical area, accompanied by severe pain, you should contact your doctor immediately.

3. Rest and Returning to Work/School

The greatest advantage of microsurgery is that the peritoneum or large muscle groups are not cut. Because the procedure is performed through the groin skin and the underlying external connective tissue, recovery is incredibly fast.

  • Resting at Home: It is best to rest at home for the first 3-4 days, spending time mostly lying down or sitting. Standing for too long may increase gravity-dependent pressure in the testicles, leading to mild aching.
  • Returning to Desk Jobs: Those who work at a computer or in jobs that do not require much physical effort can return to work in about 3 to 5 days.
  • Returning to Heavy Physical Work: For patients with physically demanding jobs such as construction, heavy lifting, or intensive sports instruction, returning to full-capacity work life should take 2 to 3 weeks.

Showering and Wound Care

In our clinic, surgical incisions are closed completely with aesthetic, self-dissolving (absorbable) hidden sutures. Therefore, there is no stressful procedure such as suture removal after surgery.

You can take a standing shower with warm water 3 days (72 hours) after the surgery without scrubbing the area. If the wound area is protected with waterproof bandages, it is recommended to keep it dry during this period. Gently patting the area dry after showering is sufficient.

4. Sexual Intercourse and Masturbation After Varicocele Surgery

This is a critical issue that patients often ask about with hesitation, but needs to be clarified regarding infertility treatment.

It is a medical necessity to abstain from sexual activity for a certain period in order to allow the internal and external tissues to heal, the ligated veins inside to close completely, and to prevent the testis from being exposed to trauma.

  • Duration: It is strictly forbidden to engage in sexual intercourse or masturbate for the first 15-20 days after surgery.
  • Why?: Physiological contractions (pelvic floor spasms) during ejaculation and the sudden pumping of blood to the testicles during the orgasm reflex can cause bleeding (hematoma) in the fresh surgical site or lead to the failure of the ligated capillaries.
  • After the 20th Day: By the end of the third week, the patient can gradually return to his normal sexual routine. Ejaculation allows the clearance of old fluids, making way for the storage of new, fresh, and high-quality sperm produced following the surgery.

5. Sports, Exercise, and Physical Activities

In the early postoperative period, any activity that increases pressure on the testicles will cause aching.

  • First Week: Short walks only.
  • Weeks 2 to 4: Light-tempo walking, swimming (after the 3rd week when the sutures are completely closed to avoid infection risk), and light jogging.
  • After 1 Month (4th Week): Extreme sports, weight training, heavy lifting, CrossFit, football/soccer, basketball, and combat sports, which significantly increase the risk of inguinal hernia and intra-abdominal pressure or carry a risk of direct impact to the testicles, should be started gradually and carefully.

6. Getting Results: When is a Semen Analysis (Sperm Test) Performed?

It is biologically impossible for sperm count to increase the very next day after surgery. The process of a stem cell developing, acquiring a tail, and turning into a fully mature, motile sperm (spermatogenesis) takes approximately 70-75 days (about 2.5 to 3 months) in men.

For this reason, to understand whether the varicocele surgery has eliminated the toxic effect of varicocele on the testicles and improved sperm parameters (in terms of count, motility, and morphology), at least one full cycle must be completed.

  • First Control Test: Should be performed exactly 3 months after the surgery (day 90). Improvement starts to show in 60-70% of patients in the first test.
  • Second Control Test: Performed 6 months after the surgery. The 6th month is when maximum results and the potential for natural pregnancy or IVF (fertilization) peak. If necessary, a sperm DNA fragmentation (DFI) test is also repeated during this period.

In conclusion, the recovery process after microsurgical varicocele surgery is highly comfortable and relatively pain-free. The only thing our patients need to do is strictly adhere to these rules regarding avoiding lifting, straining, and maintaining sexual abstinence in the first weeks.
